Path of Exile 2's Atlas offers four major endgame encounters: Rituals, Breaches, Expeditions, and Delirium. Delirium, a returning mechanic from the original Path of Exile, is detailed in this guide. Understanding PoE 2 Delirium and the Fog Mechanic
On the Atlas, map nodes guaranteeing endgame events display specific icons. Delirium Mirrors are marked with a distinctive black and white icon. A Delirium Precursor Tablet placed in a Lost Tower guarantees a Delirium event on that node.
Within a Delirium map, locate the multicolored, shattered-glass Delirium Mirror near your spawn. Approaching it triggers a ghostly energy effect. Entering the mirror initiates the encounter, spawning a large circle of Fog.
This Fog moves across the map; enemy difficulty increases with progression. Leaving the Fog ends the encounter and resets the map. Enemies within the Fog are stronger and have a chance to drop unique Delirium rewards: Distilled Emotions (used in crafting) and Simulacrum Splinters (for summoning the Pinnacle Boss). Fractured Mirrors within the Fog spawn additional mobs and loot.
Two random bosses, Kosis and Omniphobia, might appear. These are full bosses (not Rare enemies), so be prepared. They can also appear during the Pinnacle event.
Delirium Pinnacle Event: The Simulacrum
Each endgame event provides items to summon a Pinnacle Boss. High-tier Waystones in Delirium Fog yield Simulacrum Splinters. Accumulate 300 Splinters to create a Simulacrum for the Realmgate.
Activating the Simulacrum initiates a 15-wave encounter with increasing difficulty. Delirium bosses have a higher chance to spawn in later waves. Completing the Simulacrum rewards two Delirium Passive Skill points.
The Delirium Passive Skill Tree
Located within the Atlas Passive Skill Tree (top-right), the Delirium Passive Skill Tree (distinctively white, mirror-shaped) modifies Delirium events. It features eight Notable nodes and eight nodes increasing Simulacrum difficulty.
Each Simulacrum completion grants two passive points; therefore, increase Simulacrum difficulty with each new Notable node acquired.

PoE 2 Delirium Event Rewards
Delirium Fog-affected enemies drop Distilled Emotions. Bosses also frequently drop them. These Currencies anoint Amulets with Notable Passive Skills, eliminating the need for passive skill point investment. They can also add guaranteed modifiers to Waystones, increasing Delirium mob power.
Simulacrum Splinters create Simulacrums for the Realmgate, leading to a 15-wave Simulacrum event. Completion yields Delirium Passive points and a guaranteed unique item.
